That's up to every team. Some will communicate via IRC, other will use
mailing lists (the most common way), others might even use Web forums or
wikis.
Sending translations upstream again depends on the team. Some teams are
used to sending the files to their mailing list, where someone with
commit access to CVS/SVN collects them and commits them into the
repositories. Others might acquire write access themselves and do it
directly, etc.
Of course, if the Ubuntu translators don't have fluent communication
with the upstream translation teams for the programs they translate,
they might very well be doing duplicate work. That's why we're trying to
stress this quite a bit.
