First, "+" should always take you to the next unread message in the folder. 
What it does if you have no unread messages left in that folder is a matter
of configuration settings.

You can also choose to always start with the first unread message in any
folder.  

Both these settings are in "Settings", "Configure KMail", "Misc"

That's just a matter of threading.  You can turn off threading on a
per-folder basis if you really want to.  I wouldn't recommend it - once you
know you can just jump to the next unread message, it's easier to keep the
threading.
