Some of that is easily alleviated.  Turn off the rx setups you have in kmail, 
then setup fetchmail to do the pulls from your server(s) and place it in an 
unprivileged users Maildir.

The setup procmail to read that dir, run it through a bunch of procmail 
recipes including spamassassin, and have procmail send anything with 8 or 
more ** in the X-Spam-Level header to /dev/null, and put the good stuff 
into /var/spool/username/mail.

Then have kmail fetch what survives from there and sort it into the correct 
folders for you to read.  As a security precaution, setup an unprivileged 
user and run both fetchmail and procmail as that user.

If the lag to switch to the next folder gets to be more than 2 seconds, check 
the number of messages in that folder, and set the expiry rules to hold the 
message count down around the 15,000 to 20,000 mark and you should be able to 
tolerate the remaining lags.

I've been doing this for over a year, and it works rather nicely 99% of the 
time.
