So, the mouse is also plugged straight in?  

I want to make sure I have this right, when you bypassed the KVM, you were 
able to set the resolution, and it works.  Does it still work after plugging 
back into the KVM?

The mouse could be faulty, or if its cordless like mine, it could have 
distance issues with the receiver, or batteries.
