I can confirm. The "Current actions:" list is not being updated when new
actions are added from the "Available actions:" list. This is clearly a
bug.
If, however, you exit the "Configure Toolbars" window and bring it up
again, the list of "Current actions:" is updated.
Also, if you add an item to the "Current actions:" list and click Apply,
the toolbar will actually reflect the added button, whereas the
"Current actions:" list is not showing the added action/button (until
you re-open the window).

You're right :)
Not that I actually use the feature myself - I just want it to be fixed
for others to enjoy. :)
