There's a good explanation at

In a nutshell it's a means for X to get monitor features and capabilities
for adaptive behavior.

One of the features gotten from monitor via DDC is its viewable X-Y size in
millimeters. This, together with what resolution we'll be running in gives
us a calculated DPI (dots per inch) density of a given X session. AFAIK, X
tries to rescale the fonts according to this calculated DPI so as to make
a given font size physically of the same size regardless of other
particulars (i.e. resolution and monitor size).
