Audio stops working

I think (but I really don't sure!) that sometimes after playing videos in
flash (youtube etc) sound stops working *globally*.

I mean, no sound at all, for example, smplayer, amarok, kaffine, vlc.

However, only KDE apps report the problem from the screenshot I attached.
Others just play with no sound.

Any ideas?
