Continued from my previous comment...

toolbars-2.png screenshot shows where the toolbar ends up on startup.
You should be able to see the toolbar handle right after the "help" icon
to the left, lots of blank space, then the new icons on the right.

Modifying the toolbar configuration like I mentioned in a previous post
brings it back to its correct position.
