I have similar problems occasionaly - several retries help. I use
WPA2-PSK. Sometimes everything works already on boot, but in lots of
cases I need to do ipdown/up or even remove module (modprobe -r ipw2200)
and reload (ipw2200).

I have checked ipw2200.sf.net and they mention this fix:

 Feb 22 2006  Changes in 1.0.13
    * Fix DHCP broken problem caused by is_multicast_ether_addr doesn't include broadcast ether address (thanks to Olivier Hochreutiner) 

Not sure if this is it, but may be... 
Regarding "unknown notification" (not sure if related), see also bug 32419 (also fixed in recent version of the driver)
