Thanks for the suggestions.

I've done as suggested - no change. I've also removed bootchart, which
was the only non standard thing here I could think of, and reinstalled
both initramfs-tools and linux-image-2.6.12-10-386. Result remains the
same. Still seeing the error messages regarding /bin/sh and ./lib/.
