Definitely a problem with your initramfs-tools. I suspect it's something
specific to your system, something that was broken locally.

Try doing "sudo apt-get --reinstall install initramfs-tools", then rerun
the dpkg-reconfigure line. You should not see the error messages
regarding /bin/sh and ./lib/.
