Your installation medium is corrupt. You will need to recreate it and
should then verify it using the check disc option from the boot menu.
If you have done this more than once already then the usb stick must be
bad and you'll need a new one.

Bug description:
USB boot drive seems to have problems.
I tried burning iso image with both Ubuntu and WinX applications.
Both failed in different ways.
I suspect the usb stick is corrupt, but I don't know why the
formatting function doesn't fix that.
I may purchase another usb stick, but it's too late to save Ubuntu
15.4. Thankfully, I can still boot into WinX - even that didn't work
at first.
I was trying to reinstall 15.4 because of the persistent BSODs (black
screens of death). I suspect I have made the matter worse, but I
couldn't find any useful guidance.
I was better off in the 'ol Win95 days - Win98 was a great upgrade.
Alas, the world has passed me by.
