Public bug reported:

USB boot drive seems to have problems.

I tried burning iso image with both Ubuntu and WinX applications. Both
failed in different ways.

I suspect the usb stick is corrupt, but I don't know why the formatting
function doesn't fix that.

I may purchase another usb stick, but it's too late to save Ubuntu 15.4.
Thankfully, I can still boot into WinX - even that didn't work at first.

I was trying to reinstall 15.4 because of the persistent BSODs (black
screens of death). I suspect I have made the matter worse, but I
couldn't find any useful guidance.

I was better off in the 'ol Win95 days - Win98 was a great upgrade.
Alas, the world has passed me by.
