  Fix Released

Bug description:
  As shown in
  <https://plus.google.com/photos/105922848292507689403/albums/5802133545673972321/5802133606303644226?banner=pwa&pid=5802133606303644226&oid=105922848292507689403>,
  the "Pick a username", "Choose a password", and "Confirm your
  password" fields have labels both inside and outside the fields.

  They would be simpler and clearer with just the labels outside the
  fields.
