As discussed on IRC, I believe that your preseeding needs to be updated
to cope with biosdevname.  Here's some advice I just posted to ubuntu-

  DHCP autoconfiguration fails on servers since 20120822 image

Status in “debian-installer” package in Ubuntu:
  Invalid

Bug description:
  Since yesterday, any time I try to run a server install in our lab,
  the DHCP autoconfiguration step fails. It's essential this works so
  that the net install can be done. Since I don't have access to the
  system due to the same failure, logs are not included with this bug.
  Some things I've noticed though in /var/log/syslog:

  WARNING **: ethtool-lite: couldn't determine MII ioctl to use for eth0

  and

  dhclient: Bind socket to interface: No such device

  I can provide more info if needed, but getting full logs is going to
  be difficult.
