Can you check permissions of your /etc/shadow file? It should look like this:

-rw-r----- 1 root shadow  783 2006-03-27 23:51 /etc/shadow

There was a bug in KDE Users&Groups config module - see bug 26175 - did you use that module? If yes, it is probably duplicate.
