There are currently 204 high importance bugs and 731 other medium bugs.
Given the size of that queue compared to the rate of bugs fixed vs. new
reports I'm not surprised this one hasn't been fixed yet.

It is likely to be fairly shallow, so if someone is interested in trying
to fix it we'd be happy to provide some guidance.  (Where “we” is
especially likely to be me this week, as I'm the current Patch Pilot.)
I'd love to help people scratch their itches.

Probably you could look at how 'bzr cat' or 'bzr log' resolves the
filename + revno to a file-id (or even how qannotate does), which both
already seem to handle this case, and take the same approach.
