Upgrading loggerhead to 1.9-rich-root

You need to run 'bzr check' after upgrading, if there are inconsistent
(revisions | parents | file refs) you need to run reconcile.

We recommend doing this before the upgrade too.

I think we should have a template for people that includes these
instructions.
