[MERGE][1.8][bug #239343] repository_implementations => per_repository

This is in response to bug #239343. Basically, it would be good to
rename all "*_implementations" tests into "per_*" tests.

1) We want to be consistent, and we currently have both styles.
2) For lack of another reason, 'per_*' is shorter, so easier to type.

I'm going to try to break this up into a patch for each test subset, so
hopefully it is easier to review.

The other thing I fixed here, is there were modules that did:

from bzrlib.tests.repository_implementations.test_repository import
  TestCaseWithRepository

However, that class is actually defined in the __init__.py (and it is
better there anyway). So I took the opportunity to update them.

Should I go through and update the copyrights on *every* file I touch?
