[MERGE] refactor of commit to walk the working inventory less

Next step in the refactoring of commit. The walking inventory was always
being walked twice: once to find the removed files and once to build the
new inventory. These loops have been combined in this change. Some more
detailed comments on why various bits of code are necessary have also
been added.
