[MERGE](trivial) Update NEWS for 0.16rc1 and reST syntax

I've been submitting some of my Approved changes, and I realized they
should be going after 0.16rc1.

While there I also realized that we have a lot of old comments that
aren't proper reStructuredText. So I went through and updated the NEWS.
It was actually kind of a fun trip down memory lane.

The other thing I cleaned up was the 'attribution' lines. Now they are
all of the form (Committer Name, #bugnumber), and put on a single line
rather than wrapping in the middle. I think this will make it possible
to even auto-parse the NEWS file if we want to generate something from it.

Anyway, I didn't think anyone really wanted to read through a 1k
mechanical diff which just adds ```` around all of the words with '_' in
them (or other reST metacharacters).

I'm posting it to the list for completeness, but I did go ahead and
submit it to the pqm. If there are any problems, I'm happy to fix them.
