<p>Once in a while that problem happens on my Xubuntu 16.04 as well.</p>
<p>Usually after I did some plugging and unplugging of USB devices
or re-plugging the phone.</p>
<p>A restart of Xubuntu fixes this - in my case every time when I
encountered that problem.</p>
<p>Irritating nevetheless.<br>
