I received this error while trying to establish a database for the
installation. mysql 5.0.75 was running ok. The installation script
opened a window and was prompting to create a database. I accepted the
option to create a database and provided the 'administrator' password to
(I assume) the root account for the local mysql instance.

Then I received a window that indicated that the database could not be
created; Retry/Fail/some other option... After two retries, I finally
selected the Fail option. Tht is when the system detected the crash.
