[Bug 490799] [NEW] A rejected reinvite remains pending causing all further reinvites to be rejected

Public bug reported:

Binary package hint: twinkle

After rejecting a reinvite (for unacceptable codecs, for example) the
'req_in_invite' object is not cleaned up, nor the ACK from the server is
processed to let this happen. The result is that all further reinvites
will be rejected with the message 'Previous INVITE still in progress'.
So it is not possible to send a fallback codec list to twinkle.

The tested version is 1.4.2. A quick patch is attached that fixes this
problem, although its correctness needs to be checked.
