First, please disable autospawn:

echo autospawn = no|tee -a ~/.pulse/client.conf

Then, restart pulseaudio:

killall pulseaudio; pulseaudio -vvvv 2>&1 | tee ~/pulseaudio.txt

Please attach pulseaudio.txt to this bug report.
