I've seen this too. I discovered in my instance that an http cacheing server
(squid) had cached the hash sum of the packages.bz2 file, and I had to issue
an apt-get update that explicitly instructed the update to bypass any
cacheing servers along the way to the host. Try the following:

$ sudo apt-get update -o Acquire::http::No-Cache=True
$ sudo apt-get upgrade
