OK, this line:

... is bad. You can't do anything with a zero-blocks disk!

But this may be why:

That is not right.

Either 512 bytes or 4 kB is normal.

Anything else, pretty much, is wrong.

Your USB connection is bad. Bad case, bad port, bad cable; I don't
know but it's bad somewhere.
