How are you trying to do that?  By doing an upgrade within a running
16.04?  Booting off a live USB image?  Something else?

That suggests you are booting off a live image.  If it won't run the
live image then you have a problem.  Did you run the Check Files
option?  In fact I think perhaps it does that itself, did you see that
going through?

If you are booting off a USB stick then I suggest re-burning with a
different stick and seeing if that helps.  It might not make a
difference but worth trying.
