I'm not sure how you managed this one, but it's *really* wrong.

The EFI partition is a fat32 partition, not ext4,, (and in this case,
the same UUID as your /boot partition.

I can't imagine an installer getting this so messed up, so I'm wondering
how much other shenanigans might have happened alone the way.
