Since the beginning of this thread something similar is my thought,
too. He wasn't using the Google web interface or a Google app, but
Kmail, while at the same time Google worked at their servers and so the
"risky" third party app Kmail was temporarily not allowed to access the
account. Isn't Google one of those providers who warn users not to use
"insecure" third party apps, instead of their web interfaces or their
mobile apps? So even without using Google's 2-step verification, users
likely need to explicitly allow usage of third party app, IOW usage of
any MUA?! It happens all the times that e.g. POP3 extensions and/or
passwords and/or authentication settings temporarily change or are
invalid during the providers "maintenances", so much likely accessing
an Google account with Kmail from time to time accidentally could be
considered an attack from somebody else.
