Need a simple New York State prescription writing app for only non-controlled drugs

Hello!
This USA app should be for Ubuntu 14.04, simple, without the 
complications associated with prescribing controlled substances.
It should seek the Internet address of the indicated pharmacy, give the 
doctor license no and email and street address, DEA no, NPI no, date, 
patient name and address, age, sex, drug name and dose, instructions for 
taking, amount to be supplied, maximum daily dose, no of refills, and 
any other dispensing instructions.
This app should be free or inexpensive.
