Ubuntu under z/VM Installation / IPL problem

My colleague has been attempting to install Ubuntu under z/VM and has created the 4 files required to IPL under z/VM CMS for the first time;

INITRD
KERNEL
PARMFILE
UBUNTU EXEC

When he attempts to IPL by issuing L C02UBU2 BY MAINT, he is prompted by the boot menu and it then goes into a disabled wait state.

The disabled wait state indicates an inability to start the subchannel to continue the IPL, does anyone have any ideas what may be causing the problem? Is there an option during installation that should have been taken? This is being IPL'd using a V7000 , not DS8000 ECKD disk.
