Upgrade 12.04 to 14.04 problems

You should change Prompt to 'lts' when upgrading from one LTS release
to another. Also, 'do-release-upgrade -d' attempts to upgrade to a
development snapshot, in this case Xenial. You probably want to try
without the '-d'.
