Ubuntu list server issue?

My email client occasionally replies to the OP instead of the list. So my
mail goes straight to the person asking the question, and never arrives in
my inbox.

When that happens, I have to send a 2nd duplicate email to the list. The
person I'm replying to gets two emails as a result. 

Very annoying.
