Thanks. kodakaio.conf looks stock - I was looking to see if it had been
modified for a specific IP address.

I don't see Bus 001 Device 011 (device `kodakaio:libusb:001:011') in
your lsusb...

Turn off the scanner/printer & turn back on, then C&P to pastebin the
last 20 or so lines of:

$ dmesg
$ cat /var/log/syslog

and

$ scanimage --list-devices
$ sudo lsusb

Note: you should be seeing something similar to the following (replacing
Epson with kodak of course):

$ scanimage --list-devices
device `epkowa:usb:002:009' is a Epson Stylus SX620FW/Stylus Office
BX625FWD/TX620FWD/ME OFFICE 960FWD/WorkForce 630 Series flatbed scanner

$ lsusb
Bus 002 Device 009: ID 04b8:085d Seiko Epson Corp. PX-603F [ME OFFICE
960FWD Series/Stylus Office BX625FWD/TX620FWD Series]
