Run dmesg before plugging it in, then plug it in and run dmseg again.
Does anything get added.  Also what does lsusb have to say after
plugging it in?

Do other usb devices work ok if plugged into the same socket?

Do the phone and usb work ok if plugged into another pc? (or the same
pc with different OS, 13.04 running from a live image for example).

startup disk creator has some issues [1].  I advise using unetbootin.
