Help with UEFI + RAID0 install

Yes it has an UEFI system. And yes, I was able to install and run it before
with 12.10. When I tried to upgrade to 13.04 it stopped running. Then I
have tried to do the same as before and start from scratch with a new GPT
partition table in 12.10 etc and cannot make it work anymore.
