Take Alexander Skwar's advise. Regarding non-Adobe readers, be aware
that there is a significant bug that affects evince with forms:

This also is apparent with xpdf, okular, and any other PDF application
that uses the libpoppler rendering library. Were I you, I'd be more
concerned as to why Ubuntu hasn't fixed the problem in 12.04 & only
upgraded poppler in 12.10 Quantal. It's the same BS as usual... ignore
LTS release fixes and only concentrate on the next 6 month release.
