Firefox and NoScript??

Top posting corrected.
The icon appears between the back and forward buttons and the address 
bar. I have other addon icons after the address bar as well.

Try going to your .mozilla folder, then firefox folder, then your 
.default folder. Look for extensions.sqlite and click on it and if it 
asks, download Sqliteman. In Sqliteman click Tables > addon and in the 
pane on the right, locate the noscript addon and see if it shows a 1 
under the 'visible' column.
Might not be a solution, but it's a place to start.
