In OOo:
File|Wizards|Address Data Source|tick 'Evolution'|Next|Field Assignment etc.

or

File|Templates|Address Data Source|

To easily match the data fields when doing labels or mail merge,
View|Data Sources. You'll then be able to the the field labels
(file_as/book_uri/full_name etc).
