You can only install the guest additions permanently once you have booted
into the installed version, not the live version. And that is only
possible when you have unmounted the cd image.
So in order to fix problem 2, you first have to fix problem 1.

In any case the topic starter is asking the right (=smart) questions and
is supplying us with adequate information. I like him already. Just change
the topic to something meaningful next time.
