Tying to get multi input in GUI. Zenity will not work?

I have always used Zenity for my GUI user input in scripts. It works great but will only let you input one type entry at a time per screen. I need to have one screen let the user check a box and enter some text (all in the same screen). Any idea on what to use? I tried Kommander but it was extremely confusing.
