BROADCOM BCM4306 - wifi stopped working

* Although the Network Manager does not say disabled, it may be possible
that it somehow still is.  The Power Management field from iwconfig says
off, and maybe should say 'managed'.  I tried 'iwconfig wlan0 up', but that
is an unrecognized command/feature.
