You have to run it from the command line.

firefox -ProfileManager

Note that if firefox is arleady open, (ie, you want to run two separate 
profiles at the same time), then you also need the no-remote switch.

firefox -ProfileManager -no-remote

Once you uncheck the "Don't ask at startup" box, you won't need to run 
this command again unless someone re-checks it.
