alsamixer (repost)

Posted this before but got no response. Did no one have any insight? 
let's  try again.

Had to reload Hardy and now the headphones can barely be heard. Speakers 
plugged into the same socket have normal sound.
Alsamixer shows full volume for master but 0 for headphones. Furthermore 
the headphone box is collapsed with no room to
increase volume. Selecting headphones and using the up arrow does 
nothing, naturally. "Dpkg -l alsa*" shows:

Am I missing some packages and why do the speakers have normal volume 
when plugged into *the same socket*? This worked
well prior to the reload.

Any additional info required?

Any pointers or ideas?
