IIRC /dev/sr0 is the CD/DVD drive and it looks to me like the media you
are trying to use to install is bad.  I don't know if Ubuntu's install
disc does the same thing as Fedora, but with Fedora it lets you do a
check of the media to see if it's good to install from.  You might want
to burn another disc, if possible or get one from somebody who's
installed it from a disc already.
