https://bugs.launchpad.net/ubuntu/+source/linux/+bug/306719 says they solved
a problem concerning the ath5k module.

Note also the last two posts: this poster had almost the same dmesg output 
as you and then somehow WiFi started working... 

Could a bug be introduced when you updated the system after installation?

If you have an older kernel to boot I should try if that helps. And running 
the live CD might also give a clue.
