Mounting windows partitions

I relied on group advice and various other sources and added the lines 
suggested to mount two windows partitions for use at Ubuntu 8.04 
startup. The windows partitions are sda5 and sda6 and they are both 
fat32. Here below is the current fstab file. Unfortunately, I get sda6 
mounted twice and no sda5, and they only occasionally appear at startup. 
Clearly I have done some terrible thing, but can someone extricate me 
from the mess? Please!
