I'm pretty sure it depends on how the page is actually opened in the HTML. To ensure that it always opens in a new tab I use the "Tab Mix Plus" extension and force single window mode.

I too use Tab Mix Plus - and that has fixed any sort of problems like what you are talking about.
I know it was happening occasionally, but forcing single window mode did correct the issue. ; )
