Does unmuting 'External Amplifier' resolve the issue? If not, try muting
'3D Control - Switch' in addition:

amixer set 'External Amplifier' on
amixer set '3D Control - Switch' off

Daniel,

        In amixer, setting 'External Amplifier' on  had no effect.
        Then, setting '3D Control - Switch' off had no effect, either.

        Although you didn't say to, I tried re-starting the computer
after each change.
Now, amixer doesn't even show Simple mixer control 'External Amplifier'
(which was
the last item.   Nor does it show PCM.  I didn't turn PCM off and I
specifically turned
External Amplifier on.

        Duh!
